CaCoon (CAreCOOrdinatioN)

CaCoon is a public health nurse home visiting program for children with special health needs. CaCoon nurses help families coordinate care for their children and get the most out of the services available to them. Children served in the CaCoon program make fewer visits to the emergency department. They also have higher rates of immunization, annual well-child, and dental care visits. CaCoon serves children ages 0-21. The most vulnerable families, such as those with a newly diagnosed infant, are prioritized for services.

CaCoon nurses partner with families to:

  • Identify the child's strengths and needs
  • Connect with healthcare and other community supports
  • Make sure the child’s health team works well together
  • Help gather information to make health-related decisions for the child
  • Identify problems and help find solutions

Eligibility

Children (from birth to age 21) with special health care needs who require related services. Children qualify for a variety of reasons and no family is alike. Please contact us to learn how we can support you.

Referral

Anyone can request CaCoon services. Referrals come from families, medical providers, educators, or other community service providers.
