Community Support Services (CSS)

Overview

The Community Support Services (CSS) team provides intensive mental health treatment to adults who have been diagnosed with a severe and persistent mental illness. We offer an array of services that primarily focus on improving the lives of the people we serve. Accessing treatment services with the CSS team begins with scheduling an initial appointment to determine what services would be most helpful. The CSS team encompasses three programs: CSS Outpatient Team, Assertive Community Treatment (ACT), and Supported Employment.

CSS Outpatient Treatment

The CSS Outpatient Treatment Team provides a blend of community and office-based services for participants living with a serious and persistent mental illness. Our focus is to help people meet their goals. This team is made up of therapists, case managers, and peer support specialists. Services include individual therapy, groups, outreach, case management, skills training, peer support, supported employment, home and community based services under Medicaid funding, family psycho-education, medication management training, residential services, and care coordination.

Assertive Community Treatment (ACT)

ACT is an evidence-based program that provides intensive, community-based treatment using client-centered approaches to persons living with a serious and persistent mental illnesses and co-occurring issues. The goal of ACT is to keep participants housed, employed, out of jail and the hospital, and integrated into the community at the highest level of independence. It is a multi-disciplinary team that includes therapists, case managers, substance abuse specialists, peer support specialists, supported employment specialists, a nurse, a psychiatrist and a team leader.

Supported Employment 

Supported Employment (SE) helps participants find and maintain meaningful jobs in the community or to find educational opportunities. The goal of SE is to assist individuals in obtaining competitive employment in the community. SE assist with job preparation and job search, skills training for employment, as well as job development with various agencies to identify and create partnerships with local employers. SE respects the individual’s preferences and decisions about work and support. SE services continue after a suitable job is found and for as long as individuals want assistance.
